The tool uses the Bristol Stool description chart and tracks continence, urgency, comfort level, time taken, smell, colour, unusual features, external issues, action taken prior and post, images and family members. Non-verbal family members can also be tracked. It was designed for all ages and abilities in mind, including babies. It is perfect to share with your dietitian or doctor and comes with instructions. The template can be duplicated and used to track multiple family members. It also links to your Family Hub for ease of access and organization. Designed by a nurse and Notion lover!
