BenefitMall Submission Process – 51 to 100 Lives

This document outlines the eligibility, submission requirements, and process for submitting groups sized 51–100 Full-Time Equivalents (FTEs) to BenefitMall. Group size is based on FTE count, not just enrolled employees.

1. Eligibility & Required Information

Standard Rate Carriers (No Medical Underwriting)

The following carriers provide standard rates without medical underwriting:

• Anthem
• Allstate (National General)

Required for All Underwritten Submissions

1. Full Census File:
   • Date of Birth (DOB)
   • Gender
   • Home ZIP Code
   • Work ZIP Code
   • Plan Election (if applicable)
   • Tier Election (EE, ES, EC, FF, W, COBRA)
   • Must include all eligible employees
2. SIC Code
3. Current & Renewal Rates (must be on carrier paper)
4. Current & Proposed Plan Designs (carrier paper not required)
5. Carrier History (Last 5 Years):
   • List each carrier with number of years used
6. Desired Broker Commission
