BenefitMall Large Group (100+ Lives) Submission Process
This procedure outlines the eligibility requirements, submission documentation, and process for submitting large group (100+ employee lives) medical quote requests through BenefitMall.
For up to 50 EE submissions:

1. Eligibility & Required Information
Important: Carriers do not provide base rates for 100+ FTE groups. Medically underwritten quotes require detailed and accurate documentation.
General Submission Requirements (Applies to All Carriers)
- Full Census File:
- Employee First & Last Name
- Date of Birth (DOB)
- Gender
- Home ZIP Code
- Work ZIP Code
- Plan Election (if applicable)
- Tier Election (EE, ES, EC, FF, W, Cobra)
- All eligible employees must be included.
- Claims Experience (24 months):
- Monthly detail by plan type with corresponding monthly enrollment
- High Claims Report:
- Claims exceeding $25,000 during the same 24-month period (if applicable)
- Stop Loss Experience (if applicable)
- Current & Renewal Rates:
- Must be submitted on carrier letterhead or official paper
- Current & Proposed Plan Designs:
- Must also be on carrier letterhead or official paper
- Carrier History (Last 5 Years):
- List of carriers used and number of years with each
- Waiting Period for new hires
- Employer Contribution toward premiums
