Large Group Submissions
Introduction
Submissions for groups with 150+ Employee Lives have different requirements than standard-sized groups. This document outlines the necessary steps for gathering and submitting large group information for underwriting and quoting. For the official guideline sheet, refer to:

Step 1: Collect Submission Requirements
Census Requirements:
- First Name, Last Name, Date of Birth (DOB), Gender, and Zip Code.
- Plan Election: If more than one plan has been selected, list each choice under the respective employees in the census.
- Include Waived Employees.
Submission Requirements:
- Current Rates
- Renewal Rates
- SBCs for each plan choice
- Minimum of 2 Years of Claims History (Month-to-Month)
- High Claims History ($25K+ in claims): Large claimants per each time period. Must show enrollment by month for the same period.
