This process outlines how to enroll a group for Ameritas Dental and Vision coverage. Ameritas offers flat-rate ancillary plans with no RFP required and a minimum of 3 enrolled employees. Submitting the required documents confirms the group’s enrollment.

2. Vision Coverage Selection Steps

Step 1: Select Vision Network

• VSP: Includes Costco, Walmart, Sam's Club, Vision Works (Largest Network)
• EyeMed: Includes LensCrafters, Pearle Vision, Optical
• Both networks offer online options

Step 2: Choose Deductible

• Required but does not affect rates

Step 3: Choose Frame Allowance

• $130 or $150 options available

Step 4: Select Benefit Frequency

• Every 12 or 24 months
